Qualifying for the SBB

Details: Hi jmphil4, I did not attend SBB school and passed the exam on first try, yesterday. However, I attended the SBB weekend review in Houston, studied the Technical manual, Standards, Denise Harmening's 15th ed. from cover to cover, and stayed plugged-in to this site. There is a lot of education material at this site.

Unacceptable Specimen Log

Details: It is a requirement for some accreditation groups. My director found an article that stated the quantity of unacceptable specimens (for any reason) should be 0.45% or less. She is applying this "standard" to every lab. It works fine in the Chemistry and Hematology areas because they average 10,000+ samples per month; however, the Blood Bank

Quality Structure

Details: My lab is exploring differing quality structures in hospital labs. I would be interested in knowing if your labs have a quality dept. in the lab, and if so, how many people are dedicated to the dept? Does it consist of Transfusion Medicine as well as other lab areas? Thanks!

Details: Track ordering problems - duplicate orders, give order for nurses but no prepare order for Blood Bank - education needed for staff/physicians placing orders results in delay of care Transfusion documentation - check patient flowsheets for missing information (like no 15 min vitals, missing donor #, no signed consent, etc.)

Competency Assessment

Details: Only 'tests' require the 6 elements. Splitting a platelet unit would definitely be a task. If you use a tube welder for sterile docking when preparing platelet doses, the only elements you might not use would be #5, unless you choose to make a dummy unit for splitting and involve some kind of QC on the unit and #2 if you do no testing on the

SBB and route 2 (work experience)

Positive antibody screen/ negative panel

Details: Posted September 6, 2015. there are three main reasons why this can happen: 1. antibody against a low frequency antigen present on the screening cell but not on the panel. 2. Temperature difference between the screen and the panel. Main culprit here is anti-M. 3. contamination.
